The RN will be responsible for providing proactive care coordination, enhancing documentation, and conducting health risk scoring using our AI-driven platform. Duties include engaging patients, collaborating with providers, and reducing ER visits through effective care management strategies.</p><p><br></p><p><strong>Qualifications</strong></p><ul><li>Proficient in patient engagement, care coordination, and health risk assessment</li><li>Experienced in remote patient monitoring</li><li>Strong clinical documentation and assessment skills</li><li>Excellent communication and teamwork abilities</li><li>Knowledgeable in EHR systems and technology integration</li><li>Active RN license in one or more of the following states: CA, AZ, NY, NV, NM, IL, FL</li><li>Bachelor’s Degree in Nursing (BSN) or higher, with relevant certifications (e.g., CCM)</li><li>Preferred: Specialty experience in Nephrology or Cardiology, and Spanish language proficiency</li></ul><p><br></p><p><br></p>
